Transistional Living Center

We know that making the transition from hospital to home after a lengthy hospitalization can be difficult. You may be recovering from orthopedic surgery, a stroke, or bone fractures and need extra rehabilitation in order to return to the optimal level of function. Our Transitional Living Center is here to help.

The Transitional Living Center (TLC) is designed to help guests reach their highest level of independent living. Through clinical evaluations, medical care, and therapeutic activities, our therapy professionals help guests receive the care they need to return home faster.

Services available through the Center include:

• Physical, occupational, and speech therapies
• Range of motion, strength, coordination, and endurance activities
• Self-care skills
• Training and use of adaptive and assistive equipment
• Bowel and bladder management
• Family and caregiver training, as appropriate

The Transitional Living Center is a designated wing inside Heritage Manor where guests stay in semi-private rooms. The TLC benefits from the facility's tradition of and experience in providing quality nursing care. Staff members work hard to provide for the individual needs of the residents and TLC guests.

For those who cannot tolerate a full acute rehabilitation schedule, Heritage Manor’s Transitional Living Center is an ideal choice for rehab-to-home services.
